WHAT IS OCULOPLASTIC SURGERY?

Oculoplastic surgery or ocular plastic surgery is the field of ophthalmology responsible for correcting or treating the defects of the periocular facial structures: the eyelids, the orbits and the tear ducts.

 

This area encompasses a wide variety of procedures for the treatment of congenital or acquired diseases and conditions related to the periocular structures such as bags, ectropion, entropion, eyelid ptosis, eyelid retraction, eyelid tumours and lesions, thyroid orbitopathies, facial palsy, ptosis eyebrow, aesthetic alterations, etc.

¿QUÉ ES LA CIRUGÍA OCULOPLÁSTICA?

La cirugía oculoplástica o cirugía plástica ocular es el campo de la oftalmología encargado de corregir o tratar los defectos en las estructuras faciales perioculares: los párpados, las órbitas y las vías lagrimales.

 

Esta área abarca una gran variedad de procedimientos para el tratamiento de las enfermedades y condiciones congénitas o adquiridas relacionadas con las estructuras perioculares tales como: bolsas, ectropión, entropión, ptosis palpebral, retracción palpebral, tumores y lesiones palpebrales, orbitopatias tiroideas, parálisis facial, ptosis de ceja, alteraciones estéticas, etc.

WHAT CAN WE TREAT WITH OCULOPLASTIC SURGERY?

EYELID SURGERY OR BLEPHAROPLASTY

The eyelids carry out an important task in the development of the visual function, since they protect the eyes and help with their lubrication. They can be affected by a multitude of problems that are diagnosed and treated through oculoplastic surgery.

Bags and excess skin on the eyelids are a consequence of the passage of time that greatly influence the expression of the gaze and can give an image of fatigue. Blepharoplasty is the definitive surgery to correct these age-related changes and achieve a more energetic and rested look without affecting the facial expression. The surgical procedure is based on the removal of excess skin from the upper eyelids (upper blepharoplasty) and/or the removal of the lower fat bags (lower blepharoplasty).

 

 

Malpositions of the eyelids are also a frequent cause of consultation. The drooping of the upper eyelid, or palpebral ptosis, can give a fatigued look, and sometimes cause vision loss by partially closing the field of vision. Its correction makes it possible to rejuvenate the look and achieve symmetry between both eyes. Other frequent affects on the eyelids are eyebrow ptosis (lowering of the eyebrow with respect to its natural position), ectropion (outward rotation of the eyelid) or entropion (inward rotation of the eyelid). Other diseases such as facial paralysis or facial dystonias (blepharospasm and hemifacial spasm) can cause ophthalmological problems and affect visual function.

Eyelid tumours are also common. Most are benign, such as warts or cysts. Eyelid surgery is usually the treatment of choice. However, we also often see malignant tumours (cancer) that require evaluation and treatment by an oculoplastic surgeon.

 

Blepharitis is a chronic inflammation of the eyelid rim that is associated with dry eye and causes significant discomfort. On the other hand, styes and chalazions are inflammatory cysts that sometimes require eyelid surgery or blepharoplasty.

PERIOCULAR AESTHETICS

The passage of time affects the eyelids in a very marked way, where bags, marked wrinkles, sagging skin, etc. form. All these conditions cause an appearance of ageing, tiredness and even sadness, in one of the most visible areas of the body. Because of this, many patients wish to solve their problem of eyelid bags or droopy eyelids.

The term facial rejuvenation is used to define the set of techniques to correct the manifestations of skin ageing.

Depending on the type of manifestation that will be rejuvenated, one technique or another will be used:

· Wrinkles can be treated with botulinum toxin or hyaluronic acid infiltrations. The treatment of wrinkles between the eyebrows, forehead and crow’s feet, with infiltrations to relax the muscles, can significantly enhance the look. The result is a more relaxed and rejuvenated appearance without losing the naturalness and expressiveness of the face.

· The use of botulinum toxin, in some cases, allows the lifting of the eyebrow with natural results.

· Loss of face volume or glasses can be treated with the use of infiltrations.

· Blepharoplasty surgery makes it possible to permanently correct bags and drooping eyelids.

The most frequent type of inflammation of the eye socket is thyroid orbitopathy, which mainly appears in patients who have hyperthyroidism. This pathology of the eye socket is accompanied by: exophthalmos (“jumpy eyes”), red and painful eyes, inflammation of the eyelids with the formation of very visible eyelid bags, double vision and pain when moving the eyes. The treatment of thyroid orbitopathy is long and complex. This is why, during the process, it is essential that the patient feels accompanied and advised at all times in order to treat the difficulties that accompany this disease. · ORBITAL TUMOUR

Orbital tumours can be of many types and cause different symptoms such as progressive loss of vision and exophthalmos (“bulging eyes”).

 

· OCULAR EVISCERATION | EYE PROSTHESIS

In patients with blind and painful eyes, or with a severely impaired aesthetic appearance of the eyeball, enucleation or evisceration surgeries can sometimes be performed to relieve pain and improve appearance through the use of eye prostheses.

OBSTRUCTION OF THE LACRIMAL DUCT

The most common problems are tear duct obstructions, which cause epiphora or tearing, chronic conjunctivitis, discharge, blurred vision, or infections of the tear sac (acute dacryocystitis). Obstruction of the tear duct is a very common eye disease, which can affect people of any age and sex.

 

The most frequent ophthalmological and oculoplastic surgeries performed for the treatment of tear duct obstruction are punctoplasty (opening of the tear punctum) and dacryocystorhinostomy (DCR), in cases of more significant obstruction. DCR can be done externally, intranasally or with a laser depending on the case.
